What information should I include when contacting HR, Payroll, or Benefits?

To help us assist you as efficiently as possible, please include:

  • Full name (including any previous name used during employment, if applicable)
  • Employee ID number (if known)
  • Current phone number
  • Current mailing address
  • A brief description of your question or request

How can I get a copy of a previous W-2 or pay statement?

Will I receive my Form W-2 after I leave UTHealth Houston?

Yes. Former employees will automatically receive a paper copy of their Form W-2 by mail. For tax year 2026, Forms W-2 will be mailed no later than February 1, 2027.

If your mailing address has changed since your employment ended, please contact [email protected] as soon as possible to update your address and ensure your W-2 is mailed to the correct location.

If your address has not changed, no action is needed. Your Form W-2 will be mailed to the address currently on file.

How can I obtain a copy of my Form 1095?

You can access your Form 1095 by creating an account through the Paperless Employee Portal.

How do I update my mailing address or contact information?

Please contact [email protected] to update your mailing address, phone number, or other contact information.

Will I be paid for unused vacation or sick leave when my employment ends?

Eligible employees who have a remaining vacation balance when they separate from UTHealth Houston may receive payment for eligible unused vacation hours in accordance with university policy.

Unused sick leave is not paid upon termination of employment. However, upon the death of an active employee, one-half of the employee's sick leave balance, up to a maximum of 336 hours, may be paid to the employee's estate.

When will I receive payment for my unused vacation leave?

Payment for eligible unused vacation leave is processed after separation from employment in accordance with university payroll procedures. For questions regarding a specific payment, please contact [email protected].

I no longer have access to my UTHealth Houston email address. Can I still receive assistance?
